Professor Roslyn Bill, co-founder of Aston Institute for Membrane Excellence (AIME), joins Dr Matt Derry to discuss her research into brain cell membranes in the latest Aston Originals podcast.

Water moves in and out of brain cells through tiny protein channels in the cell membrane called aquaporins. One in particular, aquaporin-4, is the focus of Professor Bill’s research.

In 2020, she was lead author on a paper published in prestigious journal Cell on how the channels open and close and how this can be controlled. Uncontrolled water entry into brain cells can occur after head trauma, causing swelling which leads to severe brain injuries of the type suffered by racing driver Michael Schumacher after a skiing accident. Finding drugs to control this water movement could lead to treatments to prevent brain swelling in the first place.

This research into brain swelling and the contribution of aquaporins led Professor Bill to research into Alzheimer’s, a common form of dementia, which is also related to the action of aquaporins. Alzheimer’s is caused by a build-up of waste products in brain cells. In a process Professor Bill likened to a ‘dishwasher for your brain’, aquaporins are responsible for clearing this waste as we sleep.

Professor Bill was selected for an Advanced Grant by the European Research Council (ERC) in 2023, which is being funded by UK Research and Innovation (UKRI). The funded project will further investigate the process, and whether it might be possible to develop a drug to boost the ‘brain dishwasher’, which could be taken to slow or even prevent cognitive decline due to ageing.

Bringing together this biological research with the polymer research of AIME, chemists like Dr Derry will help in the drug development and could also lead to totally different applications.

Professor Bill said in the podcast to Dr Derry:

“We can take the knowledge that we have of how these proteins work in cells and try and apply them to interesting applications in biotechnology. And this is where the sort of work that you (Dr Derry) do comes in, where you can develop plastic membranes, polymer membranes, and then take learning from the biology and try and make really, really good ways of purifying water, for example.”

Background

Venous thromboses have been linked to several COVID-19 vaccines

Here we describe a case of post-Moderna COVID-19 vaccination arterial infarct,

with vaccine-associated diffuse cortical edema,

that was complicated by refractory intracranial hypertension.

Case summary

24 hrs after receiving her first dose of the Moderna COVID-19 vaccine,

a 30-year-old female developed severe headache.

Three weeks later she was admitted with subacute headache and confusion.

Imaging initially showed scattered cortical thrombosis with an elevated opening pressure on lumbar puncture.

An external ventricular drain was placed,

but she continued to have elevated intracranial pressure.

Ultimately, she required a hemicraniectomy,
but intractable cerebral edema resulted in her death.

Pathology was consistent with thrombosis and associated inflammatory response.

Conclusion

Though correlational, her medical team surmised that the mRNA vaccine may have contributed to this presentation.

The side effects of COVID-19 infection and vaccination are still incompletely understood.

Though complications are rare, clinicians should be aware of presentations like this one.

Lumbar puncture revealed lymphocytosis

Broad spectrum antimicrobial coverage was started.

Her mental status acutely worsened later that evening

New left hemiparesis.

Elevated D-dimer

CSF cultures and PCR testing returned negative

Intracranial pressure remained refractory to maximal medical management,

including deep sedation, paralysis and hypothermia,

prompting a right decompressive hemicraniectomy.

The patient expired from refractory cerebral edema.

Brain autopsy showed infiltrates of CD8 and CD4-positive T-cells,

in association with intravascular thrombi

Fungal and bacterial stains were negative.

Discussion

Diffuse endothelial damage and vessel wall inflammation,

suggested an underlying pro-thrombotic state and T cell inflammatory response.

The arterial infarct in this case is likely related to a pro-thrombotic state

Due to the large volume of infarction, the patient was not started on anticoagulation after extensive discussion of the risks vs potential benefit.

In summary, administration of COVID-19 vaccine was considered a possible cause of the extensive multifocal arterial thromboses with associated inflammatory response and elevated intracranial pressure given the temporal association.

The mechanism for initial elevation in intracranial pressure is not known but may relate to cerebral autoregulatory changes in the setting of cortical microvascular thrombosis.

Suspected Unexpected Serious Adverse Reaction (SUSAR) such as this would have put a pause on the entire study and an investigation into why this happened and a call for risk mitigation measures to prevent the same complication from happening to more subjects.

Gender Ideology cannot be taught at schools in the UK / Biology, Genetics & Education

Education Secretary Gillian Keegan says “contested” views on gender should not be taught as “fact” in schools. The Department for Education (DfE) will publish draft guidance on relationships, sex and health education (RSHE) on Thursday.

Teachers in English schools will not be allowed to teach children that they can change their gender identity, and age limits are also set to be imposed for the first time on when children can be taught sex education.

Ms Keegan said “the protected characteristic of gender reassignment” and being transgender should be taught and is “something adults can do” – but she said it is “different from gender ideology” and the “madness” of the “spectrum of gender” that “could change daily”.

“You can’t say this is something that people believe and people believe is a fact when it’s not,” she said.

Teaching unions say the review was “politically motivated”.

Prime Minister Rishi Sunak said the new guidance would ensure children were not exposed to disturbing content.

“Return To Normality!” Schools Banned From Teaching Gender Ideology As ‘Fact’

Backlash from parents has forced the education secretary to crackdown on how sex education is taught. Gillian Keegan has made clear in new guidance this week, that gender ideology, people being born in the wrong sex, must be taught as a ‘contested belief’ rather than a ‘fact’.

Additionally, schools will be required to share all educational materials with parents to gain their approval of the content being taught. The new guidelines also ban schools from teaching sex education to children under the age of nine.

Talk’s Julia Hartley-Brewer is joined by Conservative commentator Benedict Spence to discuss the new guidelines. Benedict says: “This is a return to normality. It shouldn’t have to be cheered as a wonderful victory, but it is.”
